I am clified as an Inependent Contractor. My income has dropped by 90 pct. Am I eligible for State Aid?

My employer says I'm' an Ind. Contractor. With the economy work is slow and my income is about 10pct of what it used to be. I have applied to the state for temporary istance until I can gain FT work however the company refuses to complete the verification forms as I am not an employee. Can I get state aid? Can they refuse to complete this form?
