Translocations in certain types of cancers - do they arise somatically? How (somatic cells - no meiosis)?

I know translocations occur as errors in chromosomal pairing before crossing over. So why would this occur during the person's life time in somatic cells? Cancers such as CML and MDS - when do the ociated translocations arise?
